This content update includes the new CPT codes effective January 1, 2021. 

New codes included:

Code:  99439

Short:  Chronic care management services, each additional 20 minutes of clinical staff time per month

Long:  Chronic care management of multiple chronic conditions with significant risk of death, acute exacerbation/decompensation, or functional decline, comprehensive care plan established, implemented, revised, or monitored; each additional 20 minutes

Effective date:  1/1/2021
